Chronic Plus® is a set of services developed to revolutionise the management of people with chronic illnesses, in order to improve the patient journey and facilitate the coordination of treatment, through closer interaction between hospital and community care.

In 2016 and 2017, Chronic Plus® received the Digital Awards About Pharma as the Best Project in the “Healthcare organisation and solutions” category and in 2017, it was awarded the Innovation and Health Prize for the most innovative Digital Health solution for the management of chronic patients.

CHRONIC PLUS® CONSISTS OF A SERIES OF COMPLEMENTARY MODULES

HCS PLATFORM

(Health Care System) A CE-certified class IIa medical device that brings together all the patient’s data, making it available to both the doctor and the network of professionals who follow the patient. The platform is a Vree Health proprietary system.

It can interface with all available data sources::
Regional Health Service Information System, Hospital Information Systems, Regional and hospital centralised booking service desks, GP medical records.
It reminds the patient of the appointments in his/her Individual Care Plan:

It monitors compliance with the ICP:
by providing the patient, doctor and any other individuals involved in the management of the patient with suggestions on how to improve compliance.
It favours supportive medicine:
by combining home telemedicine, teleconsultation, telereporting and telemonitoring services.

SERVICE CENTRE

Manned by staff (including nurses) specialised in remote patient management. The Service Centre is available to provide patients and doctors with support around the clock.

It helps the patient and physician to monitor the patient’s health.
It reminds the patient of the procedures required and to be booked, thereby promoting compliance with the ICP.
It can provide tutorship to favour patient empowerment in the management of his/her illness (for example, the importance of daily exercise, etc.)

APP

The APP, which can be used by both patients and doctors, provides easy access to all Chronic Plus® services, which, among other things, allow patients to book appointments with their doctor, thereby improving the organisation of their respective diaries.

Facilitated patient and physician access to the most important types of information: Individual Care Plan, current medication, treatment compliance reminders and appointment management.
Consultation of the clinical parameters measured either manually by the patient or by home telemonitoring devices.
